- Collaborate with the interdisciplinary team, including families, to ensure effective communication and coordination of care.
- Provide education and support to clients and families to empower them to manage their care and make informed decisions.
- Build strong relationships with clients, families, and caregivers to facilitate trust and open communication.
- Maintain accurate and timely documentation in our electronic medical record system.
- Stay up-to-date with client plans of care, changes in conditions, physician's orders, and family or caregiver requests.
- Advocate for clients and families to promote their growth and development.
- Active Registered Nurse or Licensed Practical Nurse (LPN) licensure, without restrictions.
- Valid driver's license and proof of auto liability insurance coverage.
- CPR certification required but may be obtained after hire.
- COVID-19 vaccine compliance.
- Ability to lift, carry, push, or pull up to 50 pounds.
