Job Summary

Key Responsibilities
  • Conduct thorough assessments, administer medications, and provide therapies, treatments, and interventions as scheduled in accordance with the plan of care and physician's orders.
  • Collaborate with the interdisciplinary team, including families, to ensure effective communication and coordination of care.
  • Provide education and support to clients and families to empower them to manage their care and make informed decisions.
  • Build strong relationships with clients, families, and caregivers to facilitate trust and open communication.
  • Maintain accurate and timely documentation in our electronic medical record system.
  • Stay up-to-date with client plans of care, changes in conditions, physician's orders, and family or caregiver requests.
  • Advocate for clients and families to promote their growth and development.
Requirements
  • Active Registered Nurse or Licensed Practical Nurse (LPN) licensure, without restrictions.
  • Valid driver's license and proof of auto liability insurance coverage.
  • CPR certification required but may be obtained after hire.
  • COVID-19 vaccine compliance.
  • Ability to lift, carry, push, or pull up to 50 pounds.
