Senior Case Manager

The Senior Case Manager is a key member of our team, responsible for ensuring the highest quality support to individuals with autism spectrum disorder who receive Community Based Day Services. This role involves collaborating with the Team Leader to provide continuous quality improvement of individualized programs and services.
Key Responsibilities:
- Develop and implement comprehensive written therapeutic service plans/individual support plans and implementation process for all assigned consumers.
- Convey changes in consumer goals to the larger internal support team.
- Maintain member records as required, including timely and thorough completion of necessary program assessments, progress reports, and other required documentation.
- Compile review summaries for therapeutic service plans/individual support plans.
Requirements:
- Associate's Degree in a related field and 1 year of related experience or in lieu of a degree, a minimum of 3 years of related experience.
- Ability to communicate effectively in writing.
- Strongly prefer a candidate with a demonstrated understanding of and competence in serving culturally diverse populations.
- Ability to read English and communicate effectively in the primary language of the program to which he/she is assigned.
- Ability to multi-task with frequently changing priorities.
- Basic computer knowledge.
- Ability to work independently and as part of a larger resource team.
- Valid drivers' license and access to an operational and insured vehicle.
