Licensed Practical Nurse

We are seeking a skilled Licensed Practical Nurse to join our team at the Day Habilitation Program in Ashland, MA. As a LPN, you will provide nursing services to adults and work collaboratively with our interdisciplinary team to develop and implement comprehensive written therapeutic service plans.
Responsibilities- Monitor recommendations and provide daily nursing notes and monthly summaries as necessary.
- Promote community inclusion and self-advocacy.
- Collaborate with Team Leaders, Case Managers, and Skills Instructors to develop and implement DHSP/ISP curricula.
- Administer over-the-counter and prescribed medication as necessary.
- Provide emergency first aid and direct staff in the case of a medical crisis.
- Graduate of an approved nursing school with current licensure in the Commonwealth of Massachusetts as a Licensed Practical Nurse.
- 1 year experience in a community healthcare setting.
- Strong oral and written communication skills.
- Basic computer knowledge.
- Valid drivers' license and access to an operational and insured vehicle.
