**Job Summary**
We are seeking a skilled Case Support Specialist to join our team at St. John's Residence for Boys.
As a critical support role, the Case Support Specialist will work intensively with the treatment team to provide direct services to assigned resident youth and their families.
**Responsibilities Include:**
- Developing and executing individualized plans for resident youth
- Providing support to assigned residents and their families to ensure successful outcomes
- Securing services and resources within the community to meet the needs of assigned residents
**Requirements:**
- Valid NYS driver's license (3-point maximum)
- Minimum BSW/BA degree in social work or related field
- Prior experience working with youth required; prior case management experience/knowledge of congregate settings a plus
