Santa Barbara Cottage Hospital is seeking a dedicated professional to support its Advance Care Planning Program, focusing on serving the Spanish-speaking population. As an End of Life Care Specialist, you will play a crucial role in facilitating advance care planning for patients and their families, processing advance care documents received from community members, developing educational materials for hospital staff and medical providers about advance care planning, and attending community meetings to promote advance care planning awareness.

This position requires strong communication and interpersonal skills, with the ability to work effectively with diverse groups. The ideal candidate will possess a CALIFORNIA RN LICENSE and a minimum of one year's experience in serious illness or hospice care, including end-of-life discussions with patients and family members. FLUENCY IN SPANISH IS REQUIRED, and the ability to obtain interpreter certification within one year of assuming the role is expected. Additionally, ELNEC CERTIFICATION AND POLST TRAINING ARE PREFERRED.
