Job Title: Primary Care Psychiatric Nurse Practitioner

We are seeking a highly skilled and compassionate Primary Care Psychiatric Nurse Practitioner to join our team at Southwest Care Center in Santa Fe, NM.

Job Summary:

The Primary Care Psychiatric Nurse Practitioner will provide high-quality, patient-centered care to individuals with mental health conditions, working collaboratively with our multidisciplinary team to ensure comprehensive care.

Responsibilities:
  • Conduct comprehensive psychiatric evaluations, including assessing mental status, current and past histories, and developing treatment plans.
  • Prescribe psychotropic medications and related treatments, ensuring patient safety and adherence to practice guidelines.
  • Collaborate with patients, families, and healthcare teams to provide education and support, promoting patient empowerment and self-management.
  • Participate in multidisciplinary team meetings to discuss patient care and develop treatment plans.
  • Document patient information accurately and maintain up-to-date records, adhering to governing agency and SCC policies.
Requirements:
  • Certified Psychiatric Mental Health Nurse Practitioner (PMHNP) with 3+ years of experience in behavioral health or psychiatric treatment environments.
  • Bi-lingual in English and Spanish highly preferred.
  • Current Controlled Substance Registration Certificate from the U.S. Drug Enforcement Administration and the State of New Mexico.
  • Current nurse practitioner license awarded from the State of New Mexico.
  • Basic Life Support (BLS) certification.
  • Valid NM driver's license and reliable personal transportation.
