Per Diem Licensed Practical Nurse

The Home for Little Wanderers is seeking a Per Diem Licensed Practical Nurse to join our team in the YCCS Program. As a key member of our interdisciplinary team, you will provide high-quality nursing care to youth in our crisis stabilization program.
Key Responsibilities:- Administer medication and provide nursing care to youth in our crisis stabilization program.
- Develop and implement individualized health services plans using a collaborative, wellness-focused approach.
- Assist with filling physician's orders, administering medications, and monitoring vital signs.
- Prepare leave of absence medications and/or transfer/discharge medications.
- Provide leadership on the shift by supporting the implementation of daily routines to ensure a safe and recovery-focused environment.
- Assist in making decisions regarding the course of treatment and monitoring the effectiveness, side effects, adverse reactions, and drug interactions of all medications.
- Implement comprehensive health/wellness programming, including environmental, nutritional, exercise, and fitness initiatives in collaboration with a multidisciplinary team.
- Complete reports and documentation needed for youth records, home, collaterals, and regulatory agencies.
- Assist in the accurate receipt of medication and medical supplies.
- Conduct health screenings upon intake and as necessary, ensuring all immunizations are current.
- Assist with medical, diagnostic, and laboratory appointments and initiate appropriate action.
- Contribute to the nursing assessment as part of the comprehensive assessment process.
- Provide ongoing assessment of youth, including vital signs as prescribed.
- Provide brief crisis counseling as needed and assist with crisis prevention planning.
- Ensure compliance with all regulatory guidelines.
- Assist in the delivery of health care with ancillary staff, family members, guardians, psychiatrists, primary, and specialty health care providers, and other professionals within the Home and with external organizations.
- Work actively as part of an interdisciplinary team to provide quality care, attend team meetings, and communicate regularly with team members.
- Assist with discharge planning.
- Two years of pediatric/adolescent and/or psychiatric nursing experience preferred.
- Must maintain an active LPN license in Massachusetts.
- Certified in CPR and Standard First Aid.
- Computer literate, including Microsoft Office and Internet Explorer, with the ability to learn new software applications.
