Nurse Care Manager

Hessco Elder Services is seeking a skilled Nurse Care Manager to join our team. As a Nurse Care Manager, you will be responsible for conducting nursing assessments and developing care plans for consumers referred for personal care. You will also participate in the interdisciplinary review of consumer records and provide consultation to care managers and other staff members regarding clinical issues and community resources.
Key Responsibilities:- Conduct nursing assessments and develop care plans for consumers referred for personal care
- Participate in the interdisciplinary review of consumer records and provide consultation to care managers and other staff members regarding clinical issues and community resources
- Collaborate with care managers to promote cost-effective community service plans
- Screen consumers for Frail Elder Waiver eligibility and enhanced Home Care service programs
- Screen consumers in the nursing home to assess clinical eligibility under Mass Health regulations
- Review the initial assessment and service plan for all new Home Care consumers and make recommendations related to medical needs and special programs
- Perform clinical assessments and screenings for Home Care consumers including nursing homes as needed by supervisor
- Develop and implement training presentations on medical issues for staff and outside agencies
- Valid Massachusetts RN license
- Bachelor of Science degree in Nursing or an Associates of Science degree in Nursing and at least one year clinical experience and/or nursing in the community or long-term care setting
- Valid driver's license, travel required
- Strong communicator, both verbal and written
- Proficiency in Microsoft Office, Excel, Word; able to learn new software programs
- Self-starter, able to prioritize, meet deadlines and manage time effectively
- Candidate will possess strong work ethic with a high level of integrity and good interpersonal skills
- Access to secure internet connection for remote work required
