Job Summary:The LVN will provide high-quality, patient-centered care to individuals in their homes, working closely with our interdisciplinary team to develop and implement personalized care plans.
Responsibilities:- Deliver and coordinate quality patient care in accordance with physician orders and established care plans.
- Conduct ongoing assessments and evaluations to identify patient needs and report changes to the supervisor and/or physician as necessary.
- Document all patient interactions, assessments, and care provided in a timely and accurate manner.
- Participate in the development and implementation of patient care plans, ensuring effective communication and collaboration with the healthcare team.
- Provide education and support to patients and their families on disease management, self-care techniques, and prevention strategies.
- Maintain knowledge of current nursing practices, policies, and procedures, and adhere to Aveanna's standards of care.
- Graduate of an accredited school of nursing.
- Current, unrestricted state license as a Licensed Nurse in the state of practice.
- Current CPR certification.
- Demonstrated proficiency in clinical assessments, documentation, and compliance with nursing care and policies and procedures.
- One year of experience required working under current nursing license (varies by state).
- Six months of recent experience as a Licensed Nurse in a clinical care setting.
- Home health experience.
- Ability to speak, write, read, and understand English.
- Ability to travel.
- Ability to lift 50 pounds.
- Ability to reposition patients and move equipment without assistance.
- Ability to function in a wide variety of environments, including exposure to allergens and other conditions.
- Possible exposure to blood, bodily fluids, and infectious diseases.
