Job Title: Community Behavioral Health Worker

We are seeking a dedicated and compassionate Community Behavioral Health Worker to join our team at Integrated Services for Behavioral Health (ISBH). As a member of our team, you will play a vital role in helping individuals and families achieve permanent housing stability through federal, state, and local resources.

Job Summary:

The Community Behavioral Health Worker will assess the strengths and needs of referred individuals and families, providing home and community-based behavioral health services that are practical, helpful, and collaborative. This role requires a high degree of cultural awareness, experience working with multi-need individuals and families, and a strong understanding of community service systems.

Key Responsibilities:
  • Assess and define the strengths and needs of referred individuals and families
  • Provide home and community-based behavioral health services that are practical, helpful, and collaborative
  • Meet people wherever they may be with supports for self-management of health and well-being
  • Carry out utilization review and quality assurance activities as directed
  • Maintain necessary documentation and participate in program evaluation
  • Attend team and program planning meetings, cross-systems training, and acquire knowledge of community resources
  • Meet billing productivity requirements established by Integrated Services for Behavioral Health
Requirements:
  • Minimum three years of directly relevant experience or Bachelor's degree preferred
  • High School Diploma
  • Demonstrated high degree of cultural awareness
  • Experience with multi-need individuals and families
  • Experience serving individuals experiencing homelessness, preferred
  • Personal lived experience with homelessness, preferred
  • Broad knowledge of community service systems
  • Licensed and able to operate a motor vehicle with appropriate level of insurance coverage
  • Willing to participate in and lead cross-systems team building activities
  • Able to effectively communicate through verbal/written expression
  • Must be able to operate in an Internet-based, automated office environment
