Job Summary

We are seeking a highly skilled and compassionate Diabetes & Nutrition Education Program Instructor to join our team at Family Health Services. As a key member of our healthcare team, you will provide individualized Diabetes Self-Management Education/Training (DSME/T) and Medical Nutrition Therapy (MNT) to patients, promoting healthy lifestyle choices and improving health outcomes.

Key Responsibilities
  • Deliver team-based care in our clinics, adapting to changing priorities and managing multiple tasks effectively.
  • Develop and implement comprehensive education plans, including assessments, learning objectives, and follow-up strategies, to support patients in achieving their health goals.
  • Evaluate patient progress, document outcomes, and communicate with the medical team to ensure seamless care coordination.
  • Collect and analyze data to inform Continuous Quality Improvement (CQI) efforts and maintain ADA recognition.
  • Provide education and resources to medical personnel, promoting evidence-based care and patient-centered approaches.
  • Utilize registries, electronic reports, and provider schedules to proactively assess and coordinate patient care, ensuring timely interventions and improved outcomes.
  • Follow up with patients as requested by providers, addressing barriers to care and promoting patient engagement.
  • Assist patients and families in navigating the healthcare system, accessing community resources, and adopting healthy behaviors.
  • Make reminder calls for patient visits, adhering to FHS guidelines for PCMH and Meaningful Use.
  • Participate in FHS meetings and committees, contributing to a culture of safety and excellence.
Requirements
  • Maintain current, unrestricted professional licensure as a Registered Dietitian Nutritionist (RDN), Registered Nurse (RN), or Certified Exercise Physiologist (ACSM-EP).
  • Certified Diabetes Care and Education Specialist credential is preferred.
  • Excellent verbal, written, and presentation skills, with proficiency in Microsoft Office applications.
  • Spanish literacy is a plus.
