About the Role

At UnitedHealthcare, we're simplifying the healthcare experience, creating healthier communities, and removing barriers to quality care. As a RN Nurse Care Coordinator, you'll play a vital role in our Integrated Care Model, ensuring members receive the best possible care and services to meet their needs.

This is a field-based role with a home-based office, requiring travel to local areas about 75% of the time based on business needs. If you reside within a 30-60 mile radius of Dallas County, TX, and can travel locally, you'll have the flexibility to work remotely.

Key Responsibilities
  • Assess, plan, and implement care strategies tailored to individual member needs, aiming for the most appropriate and least restrictive level of care.
  • Identify and initiate referrals for member transitions to community resources and provide assistance with social service programs, including financial, psychosocial, community, and state supportive services.
  • Manage the care plan throughout the continuum of care as a single point of contact.
  • Communicate with stakeholders to ensure quality coordinated care and services are provided expeditiously to all members.
  • Advocate for members and families as needed to ensure their needs and choices are fully represented and supported by the healthcare team.

Requirements
  • Current, unrestricted RN license in the state of Texas.
  • 2+ years of experience working within the community health setting or in a healthcare role.
  • Intermediate level of computer proficiency (including Microsoft Outlook, Teams) and ability to use multiple web applications.
  • Reliable transportation and the ability to travel in the assigned region to visit Medicaid members in their homes and/or other settings.
  • Access to a designated quiet workspace in your home (separated from non-workspace areas) with the ability to secure Protected Health Information (PHI).
  • Reside in a location that can receive a UnitedHealth Group-approved high-speed internet connection or leverage an existing high-speed internet service.
