Clinical Care Specialist

Capitol Home Health - Clinical/Field seeks a highly skilled Clinical Care Specialist to join our team. As a member of our clinical staff, you will provide direct patient care and work collaboratively with our multidisciplinary team to achieve positive patient outcomes.

Key ResponsibilitiesYour key responsibilities will include:
- Assessing patient needs and developing individualized plans of care
- Providing education and support to patients and their families
- Collaborating with the healthcare team to ensure seamless care transitions
Requirements
To succeed in this role, you will need:
- A current and unencumbered LVN license to practice professional nursing in the state of employment
- Associates' degree in Vocational/Practical Nursing from an accredited program (preferred)
- Two years of recent nursing experience, preferably in community/home health or medical/surgical settings
- Possession and maintenance of valid CPR certification
- Demonstration of excellent verbal and written communication, organization skills, and ability to work independently
Salary and Benefits
We offer a competitive salary range of $75,000 - $90,000 per year, depending on experience. You can also expect:
