We are seeking a highly skilled and compassionate Complex Care Nurse to join our team at Medix. As a key member of our interdisciplinary team, you will be responsible for delivering high-quality, patient-centered care to our frail older adult population.
Key Responsibilities- Interdisciplinary Team Collaboration: Participate in regular team meetings to contribute insights from assessments, care plan recommendations, and care coordination.
- Nursing Assessments: Conduct face-to-face nursing assessments in various settings, including home, day center, virtual, and long-term care facilities.
- Care Delivery: Deliver personalized care for a panel of patients based on care plans, in partnership with a medical provider.
- Nursing Interventions: Deliver and document nursing interventions as agreed upon in patient care plans, promptly and accurately responding to physician orders and administering medications and therapeutic interventions.
- Case Management: Provide case management during transitions of care and proactively communicate with hospital and long-term care facility case managers for care coordination.
- Patient Education: Educate patients, caregivers, and team members on how to personalize and carry out care plans.
- After-Hours Calls: Remotely take after-hours calls that are triaged by a nurse team on a rotating schedule.
- Passion for Serving High-Risk Seniors: Demonstrate a passion and mission orientation for serving high-risk seniors and frail older adults.
- Clinical Experience: Minimum 2 years of experience clinically caring for medically complex or older adult populations preferred.
- Active Listening and Conflict Management: Possess active listening and conflict management skills.
- Clinical Acumen: Demonstrate strong clinical acumen in providing clinical interventions to complex patients.
- Education and Certifications: Active, unrestricted RN license in California, graduate of an accredited School of nursing, BSN strongly preferred, CPR and First Aid certification required or requirement to obtain within 30 days of employment.
