Medical Care Specialist

We are seeking a compassionate Nurse Practitioner to join our team in Louisville, KY and Southern IN regional areas.
Job Description- Provide evidence-based medical care to seniors and individuals with complex needs, collaborating with various healthcare professionals to develop comprehensive care plans.
- Examine and treat chronic and urgent episodic illnesses, minor injuries, accidents, and other injuries or illnesses.
- Order and interpret laboratory and diagnostic testing as needed.
- Collaborate with caregivers and other healthcare professionals in providing interdisciplinary care and maintaining a comprehensive plan of care.
- Perform routine check-ups, examinations, and treatments for patients.
- Develop and implement personalized care plans tailored to each patient's unique needs.
- Collaborate with other healthcare professionals to ensure seamless care transitions and coordination.
- Maintain accurate and up-to-date patient records, adhering to HIPAA regulations.
- Active state Nurse Practitioners license.
- Current Nurse Practitioner National Board Certification.
- Active state DEA licensure.
- Master's degree in Nursing (MSN).
- Five or more years of nursing experience; at least two years of nurse practitioner experience preferred.

Benefits- Competitive compensation: $95,000 - $130,000 per year.
