Job Title: Physical Therapist

CVHCare, a leading provider of home health clinical services, is seeking a skilled Physical Therapist to join our team in San Ramon, CA.

About the Role

As a Physical Therapist with CVHCare, you will work collaboratively with patients, doctors, and support systems to provide high-quality care and support patients in achieving their health and activity goals.

Key Responsibilities
  • Evaluate patients' mobility, strength, and level of function to establish goals for improvement.
  • Develop and implement personalized therapy plans to enhance balance, walking, strength, and patient education.
  • Assess the home environment to ensure safe mobility and recommend equipment to enhance therapy.
  • Work independently to set schedules and prioritize tasks, with the support of an established industry leader.
Requirements
  • Minimum one year of experience working as a Licensed Physical Therapist.
  • Current/active Physical Therapy License – State of California.
  • BLS Certification.
  • Strong attention to detail for chart documentation.
  • Excellent time management and calendaring skills.
  • Strong experience using computers and computer systems.
