Responsibilities

As a Home Health Physical Therapist, your key responsibilities will include:

  • Providing physical therapy services to patients in their homes
  • Developing and implementing personalized treatment plans
  • Educating patients and their families on home exercise programs and proper techniques
  • Monitoring and documenting patient progress, adjusting treatments as needed
  • Collaborating with other healthcare professionals to ensure comprehensive care
Requirements

To be successful in this role, you will need:

  • Certification: Licensed Physical Therapist with no restrictions
  • Experience: At least 1 year in a similar role is highly preferred
  • Skills: Strong interpersonal and communication skills, ability to work independently and as part of a team, proficiency in therapy documentation
