Role Overview:

The Advanced Practice Providers within our Incidental Medical Services (IMS)-licensed Programs are essential in delivering high-quality care to our clients. This position involves evaluating medical appropriateness for treatment, conducting health screenings, performing physical examinations, and monitoring clients for any potential medical issues that may hinder a safe stay at the facility. Additionally, the role includes prescribing withdrawal management medications, including medication-assisted treatment (MAT), while conducting patient rounds as necessary to ensure safe stabilization. Collaboration with the Medical Director is crucial for referring clients for medical care when needed.

Specific Duties:

  • Interpret vital signs collected by LVN and/or other healthcare assistants within their scope of practice.
  • Review medical history and physical information, including communicable diseases from intake, to inform quality clinical care during treatment.
  • Analyze substance use disorder history alongside drug test results to ensure safe stabilization in treatment.
  • Complete all documentation accurately and promptly.
  • Monitor residents for signs of mental or physical deterioration that may necessitate a higher level of care, consulting with the Medical Director as needed.
  • Utilize withdrawal assessment scales (CIWA and COWS) to safely manage withdrawal symptoms through appropriate medication prescriptions.
  • Assist in gathering treatment appropriateness information for the Medical Director when required.
  • Demonstrate knowledge of the pharmacological and physiological effects of alcohol and other commonly abused substances.
  • Prescribe withdrawal management medications, including MAT, under the supervision of a physician, and perform medical tests while documenting patient health conditions.
  • Report all medical incidents and unusual occurrences to the Program Manager, Compliance Manager, and Medical Director.
  • Collaborate closely with HSI staff and referral resources to ensure quality patient care, functioning as a member of a multidisciplinary treatment team.
  • Maintain accurate and up-to-date records in electronic health systems to ensure treatment plans and progress notes are complete.
  • Communicate with clients regarding their care as necessary.
  • Provide support in cases where deviations from medication orders are requested.
  • Support program operations by mentoring other healthcare provider staff.
  • Document health-related progress notes during patient rounds.
  • Foster positive working relationships with all staff members, agency, and county contracts.
  • Maintain regular attendance and punctuality in performing scheduled duties.
  • Participate in all staff meetings and required agency meetings unless excused.
  • Ensure a clean environment by adhering to best practices.
  • Perform other duties as assigned by the HSI Medical Director and senior leadership team.
  • Review LVN/LTP documentation and enhance LVN and RN understanding of IMS services.

Qualifications:

  • Current NP-PA Licenses in the state of California.
  • Preferred: 6 months to 1 year of experience in substance use disorder, mental health, or a related field.
  • CPR certification current or obtainable within the first month of hire.
