Job Title: PRN Licensed Practical Nurse LPN

We are seeking a skilled and compassionate PRN Licensed Practical Nurse LPN to join our team at Bethany Village Graceworks. As a PRN Licensed Practical Nurse LPN, you will be responsible for providing high-quality nursing care to our residents, working closely with our interdisciplinary team to ensure the best possible outcomes.

Responsibilities:
  • Assess, plan, implement, and evaluate the nursing care for a caseload of residents from admission to dismissal on assigned nursing unit.
  • Coordinate and supervise the nursing care delivered by nursing assistants working under Team Leader's supervision.
  • Administer and chart medications and treatments, adhering to all Pharmacy procedures.
  • Make rounds on each resident, assessing for significant changes in their condition.
  • Supervise nursing assistants on the unit, evaluating work performance in terms of resident care, staff relations, efficiency of service, and intervening when needed.
  • Initiate and complete all necessary documentation and reporting.
  • Communicate all order and status changes to the interdisciplinary team.
  • Responsible for completing nursing notes according to Medicare/Medicaid guidelines and Bethany Village charting guidelines.
Qualifications:
  • License Practical Nurse from accredited school of nursing.
  • Long term care experience preferred, but not required.
  • Due to our federal funding, we are a drug-free workplace (including marijuana). A pre-employment drug screen is required for all positions.
