We are seeking a skilled and compassionate Licensed Vocational Nurse (LVN) to join our team at True Vine Concepts LLC. As a Home Health Care Professional, you will be responsible for providing high-quality patient care in the comfort of their own homes.
Key Responsibilities- Administer IVs, change dressings, and clean wounds as part of our home health care services.
- Assess and chart patient observations, including vital signs and medication reviews.
- Complete evaluation tasks, including reviewing medication and vital signs.
- Administer physician-prescribed medication and provide education to patients and families on proper home health care procedures.
- Coordinate with healthcare providers, physical therapists, and occupational therapists to ensure comprehensive patient care.
- Provide palliative care to keep patients comfortable and recommend tools or devices to improve their quality of life.
- LVNs must have passed their state board test, and we are willing to train new graduates.
- RNs must have an active license and at least 6 months to 1 year of experience in home health evaluations.
- Specialties preferred: medication management, post-arterial follow-ups, wound care management, and home health care.
- Must be willing to commute to patient homes and have a valid driver's license.
- Bilingual (Spanish/English) is preferred, but not required.
- 6 months to 1 year of home health experience is preferred.
- Detail-oriented, with a strong work ethic and commitment to patient care and schedule.
$55.00 per hour. Medical specialties: cardiology, home health, and wound care. Weekly schedule: Monday to Friday, with weekend availability. License/Certification: LVN (preferred), BLS Certification (preferred), and RN License (preferred). Work Location: On the road.
