Job Overview

We are seeking an experienced Registered Nurse to join our team as a Nursing Care Coordinator at Sholom Community Alliance. In this critical role, you will be responsible for coordinating patient care, ensuring seamless transitions, and promoting resident satisfaction.

Key Responsibilities:

  • Coordinate patient care activities, including medication administration, treatment planning, and resident assessments.
  • Collaborate with interdisciplinary teams to develop and implement care plans that meet individual resident needs.
  • Communicate effectively with residents, families, and healthcare professionals to ensure optimal care delivery.
  • Serve as a liaison between nursing staff, medical directors, and other departments to facilitate smooth care coordination.
  • Develop and maintain accurate records of resident care, including progress notes and medication administration records.
  • Participate in quality improvement initiatives to enhance resident outcomes and satisfaction.

Requirements:

  • Current Registered Nurse licensure in the State of Minnesota required.
  • Minimum 1 year of experience in long-term care setting preferred.
  • Strong communication and interpersonal skills required.
  • Familiarity with electronic health records systems and other software applications used in healthcare settings.

Compensation and Benefits:
