This is a part-time opportunity with competitive pay and benefits, including weekly pay with travel time and mileage reimbursement, holiday and PTO pay for full-time employees, and medical, dental, and vision insurance.

The ideal candidate will possess current licensure or certification as a Physical Therapist active in Arizona, one year of Physical Therapy experience, and experience working in home care settings preferred.

In this role, you will work closely with patients and their families to develop and implement comprehensive treatment plans that promote optimal health outcomes. You will assess patient status, determine the need for modifications to treatment plans, and submit documentation of treatment and skilled intervention provided.

Key responsibilities include:

  • Completing initial and ongoing comprehensive assessments
  • Assessing changes in patient status since the last visit/treatment day
  • Determining whether planned services should be modified
  • Submitting documentation of treatment and skilled intervention provided
  • Participating in educating patients and their families, and other caregivers to promote patient progress toward mutually established goals

As a Home Health Physical Therapist, you can expect a challenging and rewarding career with opportunities for growth and development. We offer a retention bonus plan during two bonus periods for company field employees who are paid mileage or have a leased vehicle.

Estimated salary range for this position is $60,000 - $125,000 per year, based on location and experience. Please note that this is an estimated range and actual salaries may vary.
