As a Home Health Physical Therapist with Interim HealthCare, you will have the opportunity to make a significant impact in the lives of others through personalized, home-based therapy. You will work closely with patients to help them regain their strength, manage their pain, and reach their goals in a home setting where therapy is customized to their daily lives.

Job Highlights

  • ">
  • Complete initial and ongoing comprehensive assessments at appropriate time points.">
  • Assess changes in the patient's status since the last visit/treatment day.">
  • Determine whether the planned services should be modified.">
  • Submit documentation of treatment and skilled intervention provided.">
  • Participate in educating the patient and the family, and other caregivers to promote patient progress toward mutually established goals.">

Requirements

  • ">
  • Current licensure or certification as a Physical Therapist active in Arizona.">
  • One (1) year Physical Therapy experience.">
  • Experience working in home care setting preferred.">
  • Valid Arizona Fingerprint Clearance card.">
  • CPR Certification (demonstration course required).">
  • No Covid Vaccination Required.">
