Position Overview
The Neighborhood Center Physician acts as a healthcare professional who drives, supports, diagnoses, and addresses a variety of health-related issues for Suvida Healthcare's patients.

The provider will manage patients within a multidisciplinary value-based care framework.

Key Responsibilities
• Provide care for Suvida Healthcare's patients, focusing on outpatient medicine, continuity of care, health maintenance, improving health outcomes, and disease prevention.
• Lead the respective care team within the Neighborhood Center.
• Collaborate effectively with Suvida Healthcare's multidisciplinary care team.
• Strive to deliver 5-Star care and maintain a high level of customer service.
• Maintain comprehensive medical histories and records.
• Refer patients to specialists as necessary.
• Participate in daily and weekly team huddles to discuss patient care and manage high-risk patients.
• Influence departmental strategy and make decisions on complex issues regarding project components.

Qualifications
• Minimum of 3 years of experience, preferably in geriatrics.
• Active, unrestricted medical license in Texas or Arizona.
• Current on all required immunizations.
• Understanding of managed care and value-based care models preferred.
• Strong leadership, customer service, and communication skills.
• Proficiency in Microsoft Office Suite.
• Bilingual/Bicultural (English and Spanish) preferred.

Education and Certification
• Doctor of Medicine (M.D.) or Doctor of Osteopathy (D.O.)
• Board certification or eligibility in Family or Internal Medicine.
• Current DEA license.
