We are seeking a skilled Licensed Practical Nurse (LPN) to join our team at Kaiser Permanente's Maui Lani Medical Offices. As a LPN - Clinic I (Meds), you will provide direct and indirect nursing care in an outpatient setting, working under the direction of a Registered Nurse and medical orders of the Practitioner.
Key Responsibilities- Reviews and prepares medical records for assigned patients, initiating follow-up as requested.
- Identifies and prepares orders for patient needs, obtaining authorization from the Practitioner and accepting verbal orders per policy.
- Observes, collects, and documents new information on patients, informing RNs or Practitioners of incomplete data, new observations, or abnormal/unusual data.
- Implements basic nursing procedures per plan of care, administering treatments, immunizations, and medications as prescribed by the Practitioner.
- Provides laboratory, x-ray, and other reports to the Practitioner for review, notifying patients of normal results as directed.
- Documents implementation of orders/interventions, recording findings, instructions, and patient responses on appropriate forms or online records.
- Collaborates with RNs/Practitioners to revise care plans as needed, observing patients/families responses to care and reporting unexpected responses.
- Coordinates projects as requested, performing efficiently in emergency patient care/clinic situations and following established procedures.
- Assigns basic nursing tasks to unlicensed personnel as appropriate, providing clear directions and acting as a resource person.
- Follows discharge plans developed by Practitioners or RNs, processing and/or following up with referrals as directed.
- Manages patient vaccine data, updating the Hawaii Immunization Registry System (HIR), Electronic Health Records (EHR), and all patient and/or external records at each patient visit.
- Twelve (12) months LPN experience.
- Hawaii State Board of Nursing approved curriculum for LPN licensure.
- Licensed Practical Nurse (Hawaii) required at hire.
- Basic Life Support from American Heart Association.
