We are seeking a skilled and compassionate Registered Nurse to join our team at Doctors Community Rehab & Patient Care Center in Lanham, MD. As a Care Management Registered Nurse, you will play a vital role in providing high-quality patient care and collaborating with our interdisciplinary team to develop effective care plans.
Key Responsibilities:- Perform comprehensive nursing assessments and develop individualized care plans to meet the unique needs of each patient.
- Collaborate with the nursing team, physicians, and other healthcare professionals to ensure seamless care coordination and communication.
- Delegate patient care responsibilities to staff and supervise them to ensure that tasks and interventions are implemented effectively.
- Communicate patient information with assigned staff and between shifts to ensure continuity of care.
- Must be a graduate of an approved school of nursing and currently licensed by the State Board of Nursing.
- CPR Certification is required.
- One to three years of experience working as a RN in a related setting is preferred.
