Advanced Acute Care Nurse

Mercy's Hospital-at-Home program is seeking a skilled Registered Nurse to provide high-quality, advanced acute care in the comfort of patients' homes. As a key member of our interdisciplinary team, you will be responsible for delivering daily hospital-level patient care that aligns with Mercy's mission, culture, and standards of practice.
Responsibilities- Provide advanced acute skilled nursing care with clinical and home assessments, teaching, treatments, documentation, and coordination of care with HaH interdisciplinary clinical teams.
- Collaborate with patients, families, and healthcare providers to develop and implement individualized care plans that promote optimal health outcomes.
- Utilize strong nursing, communication, and organizational skills to prioritize and manage home-based visits efficiently.
- Work independently, be self-directed, and adapt to unpredictable circumstances while maintaining a high level of professionalism and compassion.
- Graduate of an accredited registered professional nursing program.
- Current RN license within the hiring state and/or compact licensure in which nursing duties are performed.
- A minimum of two years of previous nursing experience in an acute inpatient or emergency department setting.
- Basic Life Support Health Care Provider card or successful completion of course within 30 days.
