About the Role

We are seeking a highly skilled and compassionate MDS Coordinator to join our team at JGS Lifecare. As a key member of our healthcare team, you will play a vital role in enhancing the quality of care provided to our residents. Your expertise in nursing principles and ability to navigate complex healthcare regulations will be essential in ensuring the highest level of care.

Responsibilities
  • Conduct comprehensive assessments of residents to identify their needs and develop individualized care plans.
  • Collaborate with RNs and LPNs to ensure compliance with state and federal regulations and to develop innovative solutions to improve care.
  • Document resident progress and address any concerns that arise within the facility.
  • Participate in interdisciplinary meetings to foster a culture of continuous improvement in resident care.
Requirements
  • A strong foundation in nursing principles, requiring either an LPN or RN license.
  • Excellent organizational skills and attention to detail.
  • Ability to effectively utilize various software and tools for assessment and documentation.
  • Critical thinking and problem-solving abilities.
  • Strong interpersonal and communication skills.
