Job Overview

The Youth Support Case Coordinator focuses on providing outreach, advocacy, and comprehensive case management for young adults who have experienced sexual exploitation.

Reporting Structure: Reports to the Outreach and Support Services Manager

Work Schedule: Monday to Friday, 11:00 AM to 7:00 PM

Key Responsibilities

Community Engagement and Outreach

  1. Establish connections and build trust with young adults aged 18-24 who are currently facing or are at increased risk of homelessness, human trafficking, and sexual exploitation.
  2. Conduct outreach initiatives at various community venues, including organizations, libraries, educational institutions, and social service agencies, to raise awareness about available program services.
  3. Foster and maintain relationships with legal entities, law enforcement, and other systems that young individuals may interact with.

Direct Service Responsibilities

  1. Provide personalized advocacy for young individuals who are survivors of commercial sexual exploitation or are at risk.
  2. Create tailored, trauma-informed, and client-centered service plans addressing education, employment, health, housing, life skills, mental health, recovery, safety, and substance use.
  3. Maintain regular communication with clients to address their evolving needs and crises.
  4. Assist young adults in navigating various systems, including but not limited to detention facilities and social services.
  5. Connect young survivors to essential community resources, including education, employment, health services, and housing.
  6. Collaborate with colleagues, community partners, and family members to engage and support clients in enhancing their safety and accessing appropriate services.

Administrative Duties

  1. Ensure timely and accurate documentation of client files, including enrollment forms, service plans, and case notes for up to 15 clients.
  2. Prepare monthly reports in accordance with agency and contractual obligations.
  3. Participate in regular supervision, case consultation meetings, and team meetings.

Qualifications

a. Education and Experience

  1. A Bachelor’s degree in a human services field is preferred.
  2. At least 2 years of relevant experience providing direct services to young adults who have faced commercial sexual exploitation or trafficking.
  3. Knowledge of crisis intervention, child welfare, domestic violence, family dynamics, homelessness, and trauma-informed care.
  4. Experience in networking and advocating for young adults to access educational, financial, housing, and medical resources.
  5. Familiarity with motivational interviewing and employment coaching is preferred.
  6. Proficient in using MS365 Power Platform.

b. Additional Requirements

  1. Must pass a criminal history background check.
  2. Must pass a tuberculosis (TB) test.
  3. Reliable transportation and a valid driver's license are required.
  4. Must have a safe driving record if operating a vehicle for work purposes.
  5. Must carry auto liability insurance if using personal vehicles for work.
  6. Completion of health certification requirements, including CPR and First Aid, within 30 days of employment.
  7. Proof of COVID-19 vaccination is mandatory.
