Licensed Practical Nurse Position in Brooklyn, NY

We are seeking a highly skilled Licensed Practical Nurse (LPN) to join our team at The Allure Group's Skilled Nursing Facility in Brooklyn, NY. As an LPN with our organization, you will have the opportunity to make a significant impact on the lives of our residents and clients.
About the RoleThis position involves making regular rounds to observe residents and nursing care, reviewing and integrating the Nursing Care Plan to reflect physical emotional, restorative, rehabilitative, teaching, and goals with the medical care prescribed, and providing proper transcription and execution of physical orders. You will also utilize the capabilities of staff members to provide quality resident care, order medication and supplies for the nursing unit and residents, control the counting and recording of medications, and maintain proper and accurate documentation and maintenance of clinical records.
Responsibilities:- Observe residents and nursing care, identify areas for improvement, and implement changes as needed
- Develop and implement individualized care plans to meet the unique needs of each resident
- Collaborate with interdisciplinary teams to coordinate care and services
- Communicate effectively with residents, families, and healthcare professionals
- Administer medications and treatments as prescribed by physicians
- Monitor and document resident progress, identifying and addressing any concerns or issues that may arise
