We are seeking a qualified professional to join our Community Support Team as a Mental Health Specialist. This role involves providing community-based mental health and substance use services to adults, with a focus on structured rehabilitative interventions to increase and restore an individual's ability to live successfully in the community.
Key Responsibilities- Assist with the development of relapse prevention and disease management strategies to support individuals in achieving their goals.
- Ensure linkage to clinically appropriate and effective services, including arranging for psychological and psychiatric evaluations as needed.
- Provide intensive case management to support individuals in navigating the healthcare system and accessing necessary resources.
- Conduct functional needs assessments to determine the scope and anticipated outcomes of services.
- Assist with housing searches and connecting individuals to financial and in-kind resources to establish and maintain their households.
- Ability to perform tasks on the computer, including working with Microsoft 360, Excel, Word, and other basic programs.
- Valid driver's license, auto insurance coverage, and reliable transportation.
- Pass criminal background check and other regulatory checks.
- Provide proof of education and relevant experience.
- A bachelor's degree in Human Services, Social Work, Special Education, or a closely related field.
- For Advanced Practice (AP), one year of professional experience working with the adult mental health population.
- For Qualified Professional (QP), two years of professional experience working with the adult mental health population.
