About the Role:

As a Mental Health Professional for Care Management at A Caring Alternative, you will play a vital role in improving positive client outcomes through linking clients to medication management, coordinating primary and behavioral health care needs, building trust between clients and their practitioners, connecting clients to community resources, and enhancing communication and the continuity of care.

Key Responsibilities:
  • Completing comprehensive assessments to identify client needs and goals.
  • Facilitating case conferences to ensure effective care coordination.
  • Ensuring medication monitoring and reconciliation occur.
  • Monitoring progress toward client goals and making adjustments as needed.
  • Managing care transitions, including creating 90-day transition plans.
