We are seeking a highly skilled Certified Case Manager to join our team at Houston Methodist Cypress Hospital. Key Responsibilities- Collaborate with physicians and interprofessional healthcare teams to develop and implement patient-centered care plans.
- Conduct thorough assessments and identify opportunities for improvement in patient care and hospital operations.
- Develop and implement strategies to reduce length of stay, readmissions, and healthcare costs.
- Provide education and support to patients and families, promoting self-management and empowerment.
- Communicate effectively with payors, insurers, and other external stakeholders to ensure timely and accurate reimbursement.
- Bachelor's degree in nursing or related field.
- Master's degree preferred.
- Five years of hospital clinical nursing experience, including two years in case management.
- Current RN licensure in Texas or compact state.
- ANCC-recognized certification in case management or equivalent.
