Job Title: Licensed Practical Nurse

We are seeking a skilled Licensed Practical Nurse to join our team at Northern Light Home Care and Hospice. As a LPN, you will play a vital role in providing high-quality patient care and contributing to the success of our organization.

Job Summary:

The Licensed Practical Nurse is responsible for providing nursing care to patients in their homes, under the direction of a registered professional nurse. This role requires strong communication and interpersonal skills, as well as the ability to work independently and as part of a team.

Responsibilities:
  • Provide nursing care to patients in their homes, including administering medications, performing wound care, and providing education to patients and families.
  • Collaborate with the healthcare team to develop and implement individualized care plans.
  • Conduct thorough assessments of patients' physical and emotional needs, and document findings in the patient's medical record.
  • Communicate effectively with patients, families, and healthcare team members to ensure seamless care coordination.
  • Participate in ongoing education and training to stay current with best practices and industry developments.
Requirements:
  • Current licensure as a Licensed Practical Nurse in the state of Maine.
  • High school diploma or equivalent required.
  • Ability to work independently and as part of a team.
  • Strong communication and interpersonal skills.
  • Ability to lift, move, and load 30 to 50 pounds.
Working Conditions:

This position requires the ability to work in a fast-paced environment, with frequent interruptions and changing priorities. The LPN will be required to work in patients' homes, which may involve exposure to hazardous materials, noxious odors, and very hot or cold temperatures.
