Responsibilities:
- Provide high-quality patient care and support to clients in their homes
- Develop and implement individualized care plans to meet the unique needs of each client
- Collaborate with other healthcare professionals to ensure seamless care coordination
- Monitor and report changes in client condition to the Director of Nursing and other relevant personnel
Requirements:
- Current unrestricted RN license in the state of Georgia
- Minimum one year of experience in a home health agency, primary care clinic, or health facility
- Exceptional nursing assessment skills and analytical thinking
- Licensed driver with valid automobile insurance
Preferred Qualifications:
- Bachelor's degree in Nursing
- Certified CPR and negative TB skin test or chest x-ray
