Responsibilities:

  • Provide high-quality patient care and support to clients in their homes
  • Develop and implement individualized care plans to meet the unique needs of each client
  • Collaborate with other healthcare professionals to ensure seamless care coordination
  • Monitor and report changes in client condition to the Director of Nursing and other relevant personnel

Requirements:

  • Current unrestricted RN license in the state of Georgia
  • Minimum one year of experience in a home health agency, primary care clinic, or health facility
  • Exceptional nursing assessment skills and analytical thinking
  • Licensed driver with valid automobile insurance

Preferred Qualifications:

  • Bachelor's degree in Nursing
  • Certified CPR and negative TB skin test or chest x-ray
