Job Summary:

The Licensed Social Worker will act as a support system for individuals with behavioral health issues, maintaining a caseload that includes PH, IOP, and OP levels of care. You will work closely with our multidisciplinary team to provide comprehensive treatment plans, assessments, and interventions.

Responsibilities:
  • Teach individuals how to modify behaviors and develop coping strategies
  • Conduct assessments, identify issues, and create individualized treatment plans
  • Assist with case management and provide outreach and progress reports to referring agencies
  • Lead group therapy sessions and provide individual counseling
  • Refer patients to support groups and set up aftercare plans
  • Obtain authorizations from Medicaid, insurances, and other referral sources
Requirements:
  • Master's degree in Social Work
  • NJ Licensed Social Worker
  • At least one year of experience as a LSW
  • Knowledge of ASAM criteria and NJSAMS
