Job Summary

We are seeking a skilled Licensed Practical Nurse (LPN) to join our team. As an LPN with us, you will have the opportunity to make a difference in the lives of our clients while enjoying a flexible schedule that suits your needs.

Compensation

The estimated hourly rate for this position is between $30.00 and $32.50 per hour.

Responsibilities

As a Licensed Practical Nurse with Professional Case Management, your responsibilities will include:

  • Providing high-quality patient care in the comfort of clients' homes
  • Creating personalized care plans tailored to each client's needs
  • Maintaining accurate and detailed records of client interactions and care provided
  • Collaborating with our interdisciplinary team to ensure comprehensive care
Requirements

To be considered for this role, you must possess:

  • A current, unrestricted license as an LPN/LVN in the state(s) of practice
  • Minimum one year experience as an LPN/LVN in an acute care setting or equivalent; new graduate nurses are also encouraged to apply
  • Current CPR certification and TB test
