Licensed Practical Nurse

We are seeking a skilled Licensed Practical Nurse to join our team at Aveanna. As a LPN, you will play a vital role in delivering high-quality patient care and collaborating with our healthcare professionals to ensure the best possible outcomes.
Job Summary:The successful candidate will be responsible for providing direct patient care, administering medications, and documenting patient information. You will work closely with our interdisciplinary team to develop and implement individualized care plans that meet the unique needs of each patient.
Responsibilities:- Deliver high-quality patient care in compliance with physician orders
- Continuously observe and assess patient condition and care needs
- Document all activities, assessments, and nursing actions in a timely manner
- Participate in the development and implementation of individualized care plans
- Take appropriate nursing action based on assessment and achieve expected outcomes
- Recognize changes in patient needs and responses requiring intervention and implement care to prevent risk or reduce risk
- Accept responsibility for personal and professional accountability by complying with Aveanna policies, state and federal regulations, and the Nurse Practice Act
- Provide care utilizing infection control measures that protect both staff and patients according to OSHA standards
- Educate patients and families regarding disease processes, self-care techniques, and prevention strategies
- Graduate of an accredited school of nursing
- Current, unrestricted state license as a Licensed Nurse in the state of practice
- Current CPR certification
- Demonstrated proficiency in clinical assessments, documentation, and compliance with nursing care and policies and procedures
- Continuing Education as required by state
- Six (6) months of recent experience as a Licensed Nurse in a clinical care setting
- Home health experience
- Must be able to speak, write, read, and understand English
- Must be able to travel
- Must be able to lift 50 pounds
- Must be able to sufficiently reposition patients and move equipment without assistance
- Prolonged walking, standing, bending, kneeling, reaching, twisting
- Must be able to sit and climb stairs
- Must have visual and hearing acuity
- Must have a strong sense of smell and touch
