Job Summary

The Clinical Director is responsible for overseeing and directing the patient care operations of the home care facility. This includes coordinating and completing assigned projects to effectively support the immediate and long-range objectives of the company. The Clinical Director will oversee the eligibility of patients referred to home care services, plan for the services to be provided to patients, and supervise their total home health care. The Clinical Director will also implement and maintain administrative practices, agency philosophy, goals, and policies that assure compliance with applicable state and federal regulations. Additionally, the Clinical Director will enhance the profitability of the agency, provide motivation and retention of a qualified staff, and assure the quality of services delivered.

Key Responsibilities
  • Supervise and ensure high-quality patient care services throughout the episode of care, including referral coordination, patient needs assessment, and development, implementation, and updates of the patient's individualized plan of care.
  • Supervise patient care to ensure the delivery of safe, economical, and efficient patient care, adhering to relevant standards of accepted nursing and medical practice, including federal and state regulations, and Joint Commission standards.
  • Supervise agency staff worker performance and monitor and assess employee performance regularly using the clinician scorecard and yearly with the performance evaluation tool.
  • Ensure that only qualified personnel are hired, oversee licensure verification, certification, and credentialing as required by law and policy prior to employment and annually thereafter.
  • Assist with staff recruitment to ensure adequate staffing is maintained, operate within the staffing model.
  • Serve as a member of the Management Team and QAPI Team according to required timeframes, implement Performance Improvement Projects, and annual strategic agency planning, ensuring follow-up on any required action plans.
  • Maintain and document an infection control program with the goal of preventing and controlling infections and communicable diseases.
  • Participate in annual strategic agency planning, ensure follow-up on any required action plans.
  • Conduct at least monthly meetings with the medical director, approve medical director invoices, and submit on a monthly basis to the Home Office with appropriate supporting documentation for timely payment.
  • Drive Operational metrics, including productivity, VBP, SVP, ACH, claims alert, late EMS, unverified services, orders audit, and agency financial statements to ensure the organization is on target with company goals related to operational efficiency, growth, quality, and finances.
  • Act as Emergency Coordinator in the absence of the Executive Director/Alternate Executive Director during emergencies, ensuring appropriate plan execution.
  • Communicate effectively and professionally with all customers to represent the agency, ensuring a timely and thorough investigation of concerns voiced by patients, caregivers, and referral sources occurs, including follow-up with the complainant to ensure satisfaction with the resolution of the complaint.
  • Maintain confidential documentation within the information system, collaborate with the sales team to grow the business.
  • All other duties as assigned.
Education and Experience

License Requirements:

  • If Clinical Director is an RN, must have current RN licensure in the state of practice.
  • Current CPR certification required.
  • If Clinical Director is another clinical discipline (PT/OT/SLP), must have current applicable license in the state.
  • One year of supervisory and/or management experience in home health setting or related healthcare setting preferred.
  • Current Driver's License and vehicle insurance, and access to a dependable vehicle, or public transportation.
  • All CMS and state regulations must be followed regarding supervision of nursing services.
  • If Clinical Director is not an RN, Executive Director/Administrator or Patient Care Manager must be designated as supervising RN for nursing services unless state regulations dictate otherwise.
