We are seeking a highly skilled Clinical Pharmacist to join our team at Sigma Healthcare. As a key member of our healthcare team, you will be responsible for providing exceptional patient care and contributing to the delivery of high-quality healthcare services.

Key Responsibilities:
  • Maintain a high level of productivity and efficiency in providing patient care services.
  • Participate in peer review and performance improvement activities to ensure the highest standards of care.
  • Practice aseptic techniques and comply with infection control guidelines to prevent the spread of infection.
  • Function with awareness and application of safety procedures to ensure a safe working environment.
  • Perform efficiently in emergency patient situations, remaining calm and documenting events as required.
  • Anticipate potential problems and make appropriate interventions to ensure patient safety.
  • Apply an awareness of legal issues in all aspects of patient care and strive to manage situations in a reduced risk manner.
  • Participate in the implementation of the Family Advocacy Program as directed.
  • Exercise awareness and sensitivity to patient/significant others' rights and maintain an awareness of responsibility and accountability for own professional practice.
  • Participate in continuing education to meet own professional growth and maintain Basic Life Support certification.
  • Attend and/or comply with all annual training classes required by the Command, including disaster training, infection control, and other relevant training.
  • Actively participate in the command's Performance Improvement Program and provide in-service training to clinic staff members.
  • Perform timely, accurate, and concise documentation of patient care and operate automated systems such as electronic health records and various databases.
  • Exercise appropriate delegation of tasks and duties in the clinical direction and coordination of health care team members, patient care, and clinic activities.
  • Maintain documentation of all treatment provided in accordance with clinic directives and prepare records and reports as required.
Requirements:
  • Doctor of Pharmacy (Pharm.D.) degree from an accredited institution.
  • Graduate from a college or university accredited by the Accreditation Council on Pharmaceutical Education (ACPE).
  • Completion of a clinical pharmacy residency or fellowship accredited by the American Society of Health System Pharmacists or American College of Clinical Pharmacy, or documentation of appropriate education, training, and/or CME in the practice of clinical pharmacy.
  • Current, full, active, and unrestricted license to practice as a pharmacist.
  • One (1) year of full-time experience in the last three (3) years as a clinical pharmacist in an ambulatory care setting, or completed an ASHP accredited Ambulatory Care Residency, or possess Board Certification as a Pharmacotherapy Specialist.
