Key Responsibilities- Assess patients' discharge planning needs and develop personalized plans to ensure a smooth transition to the next level of care.
- Collaborate with the interdisciplinary team to identify and address any barriers or gaps in care that may impact patient outcomes.
- Participate in the utilization management process to ensure that patients receive the most appropriate level of care.
- Act as a liaison between the hospital and post-acute providers to ensure seamless transitions of care.
- Graduate of an accredited school of nursing with a current RN license in the state of employment.
- Three years of acute care experience, with a focus on case management.
- Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
