Job Title: Physical Therapy Assistant Home Health

We are seeking a skilled Physical Therapy Assistant to join our team at Pinnacle Home Care Inc. in Florida. As a Physical Therapy Assistant, you will work under the supervision of a licensed Physical Therapist to provide physical therapy services to patients in their own homes.

Job Summary:

The Physical Therapy Assistant will assist in implementing treatment plans designed to improve mobility, reduce pain, restore function, and prevent disability. The role involves traveling to patients' homes, delivering therapeutic exercises, and providing patient education to ensure effective and safe care in a home environment.

Responsibilities:
  • Assist in development and implementation of the interdisciplinary client care plan as it pertains to Physical Therapy.
  • Treat clients to relieve pain, develop or restore functional and maintain maximum performance, using physical means, such as exercise, massage, heat, light and electricity.
  • Teach, supervise, and counsel the family and client in the physical therapy program.
  • Instruct client in care and use of adaptive devices such as: wheelchairs, braces, crutches, canes and prosthetic and orthotic devices.
  • Arrange for out-patient service for procedures when appropriate.
  • Record on a timely basis all treatments, and client's response to therapy intervention Also record quality assurance data.
  • Communicate with PT and other team members (including Home Health Aides), when appropriate. Instruct them in certain phases of physical therapy that they may use while working with the client
  • Under the direction of a Physical Therapist, write clinical/progress notes on the date of the visit and incorporate same in the client's record according to company policy.
  • Participate in case conferences, physical therapy staff meetings, agency activities and in-service programs as necessary.
  • Participate in effective coordination of schedule to ensure all reassessments are performed in a timely manner.
  • Obtain a medical history, particularly as it relates to the present condition.
Requirements:
  • The ability to drive and the ability to access clients' homes, which may not be routinely wheelchair accessible are required.
  • Hearing, eyesight, and physical dexterity must be sufficient to perform a physical assessment of the client's condition and to perform client care.
  • May be required to bend, stoop, reach and move client weight up to 250 pounds, lift and/or carry up to 30 pounds.
  • Must be a graduate of an approved school giving a course of not less than two years for Physical Therapist Assistants.
  • Must be licensed by the State as a Physical Therapist Assistant.
  • Must have one year of experience in an acute/sub-acute setting.
