Position Summary


The primary duty of this role is to deliver compassionate care to the Elders residing in a Green House home. This involves safeguarding, nurturing, and supporting the Elders while assisting them with daily living activities and addressing their various needs.

The essential aim of the Shahbaz's responsibilities is to facilitate assistance in a way that enhances the quality of life and fosters a positive Elderhood experience.

All tasks will adhere to the existing federal, state, and local standards, guidelines, and regulations governing long-term care and assisted living facilities. You will be entrusted with the administrative authority, responsibility, and accountability necessary for fulfilling your assigned duties.


Key Responsibilities

  • Collaborate with your household team of Shahbaz to identify and respond to the needs and wishes of the Elders.
  • Encourage the independence of Elders in making decisions about their daily lives and the household.
  • Work cohesively with fellow Shahbaz as a self-directed team, managing all aspects of the home, including food procurement, preparation, and cleanup, light housekeeping, laundry for the Elders, and assisting them in engaging in preferred daily activities.
  • Partner with nursing and clinical team members to ensure the well-being of each Elder.
  • Communicate resident status at the beginning and end of each shift, as well as when necessary, to team members and nursing staff.
  • Assist Elders with personal care tasks, including dental hygiene, bathing, dressing, grooming, hair care, nail care, shaving, and other hygiene needs.
  • Support and/or execute restorative and rehabilitative procedures as specified in the care plan.
  • Measure and document vital signs such as temperature, pulse, respiration, weight, height, and other relevant metrics.
  • Assist in moving, positioning, and transporting Elders in and out of beds, chairs, bathtubs, wheelchairs, and lifts; conduct room checks to verify resident locations.
  • Help with the application of slings, elastic bandages, and other supportive devices.
  • Provide range of motion exercises as directed by the care plan.
  • Collaborate with the Elders and the Dietitian to design a balanced menu that satisfies both nutritional requirements and personal food preferences.
  • Prepare and serve meals, snacks, and other nutritional options according to the established menu and Elder preferences.
  • Maintain cleanliness throughout the home by performing routine housekeeping tasks, such as changing bed linens, emptying trash, and dusting.
  • Handle laundry duties for clothing, linens, and other items.
  • Ensure that Elder requests for assistance are addressed promptly.
  • Work alongside the Activity professional to develop and implement support for the Elders' daily activities, including opportunities for spontaneous engagement.
  • Uphold confidentiality and the rights of Elders, families, employees, and household information in accordance with PHI HIPAA policies and applicable regulations.
  • Meet regularly with the Clinical Service Team (CST) to discuss Elders' needs and progress in addressing any issues.
  • Observe, monitor, and report any changes in condition to the CST.
  • Document observed data in compliance with policy.
  • Rotate through various coordinator roles, including Team, Care, Scheduling, Food & Housekeeping Coordinator.
  • Participate in Team meetings consistently.
  • Engage in orientation programs, ongoing training, and educational sessions.
  • Maintain all equipment and appliances in the home by following proper operating guidelines.
  • Document and keep records necessary to comply with all state and federal regulations.
  • Report any equipment or appliance issues immediately to the maintenance director.
  • Adhere to all safety protocols and procedures; monitor and report any unsafe conditions to the Guide.
  • Document and report any accidents or incidents that occur.
  • Report any instances of abuse or suspected abuse to the Guide without delay.
  • Follow established smoking regulations and report any violations. (Non-smoking campus)
  • Be open to learning and performing duties beyond the traditional scope of practice of a CNA, such as cooking, cleaning, laundry, and activities.
  • Commit to upholding the core values and practices of a Green House Home.
  • Perform all other duties as assigned.
