Position Overview
We are in search of a compassionate and skilled Nurse Practitioner to provide comprehensive healthcare services to patients in long-term care settings. This role is vital in enhancing the quality of life for patients while collaborating with a committed team of healthcare professionals.

Key Responsibilities:
  • Deliver comprehensive healthcare services and coordinate patient care effectively.
  • Collaborate closely with a multidisciplinary team of healthcare providers.
  • Evaluate, diagnose, and treat both the physical and mental health needs of residents.
  • Conduct detailed patient assessments, including thorough medical history evaluations and physical examinations.
  • Order and interpret diagnostic tests to inform treatment decisions.
  • Formulate accurate diagnoses and develop tailored treatment plans.
  • Prescribe medications and administer necessary treatments.
  • Provide guidance and support to residents and their families.
  • Work with the interdisciplinary team to ensure seamless care coordination.
  • Manage chronic health conditions and monitor patient progress.
  • Adjust care plans as needed to meet patient needs.
  • Commit to delivering person-centered care.
  • Create a safe and supportive environment for elderly residents.


Qualifications:
  • Nurse Practitioner License
  • DEA License
  • Minimum of 6 months experience in long-term care settings

Salary Range: $90K - $120K per year
