As a key member of our interdisciplinary team, you will play a vital role in delivering high-quality, compassionate care to complex, chronic patients in the comfort of their own homes.
Key Responsibilities- Provide Comprehensive Care: Conduct initial comprehensive assessments, develop and implement patient management care plans, and provide ongoing care to patients with complex, chronic conditions.
- Collaborate with Interdisciplinary Team: Work closely with primary care physicians, consultants, and other healthcare professionals to ensure seamless care coordination and optimal patient outcomes.
- Prescribe Appropriate Diagnostics and Interventions: Utilize your expertise to prescribe diagnostic tests and interventions to prevent unnecessary acute admissions and promote cost-effective care.
- Stay Up-to-Date with Clinical Standards: Maintain current knowledge of clinical standards of care, CPT, ICD-10, and HCPC codes, and NCQA requirements, CMS guidelines, Medicaid/Medicare contracts, and benefit systems.
- Participate in Quality Initiatives: Actively participate in ongoing meetings and quality initiatives to ensure the highest standards of care and promote the improvement of care management and delivery.
- Current NP/PA License: Possess a current Nurse Practitioner or Physician Assistant license.
- Completion of Accredited Program: Have completed a Nurse Practitioner or Physician Assistant accredited program.
- Reliable Transportation: Have access to reliable transportation and comply with Optum's Motor Vehicle Safety policy.
- 2+ Years of Clinical Experience: Possess 2+ years of clinical experience in primary care, home-based medical care, geriatrics, or palliative care.
- Knowledge of Long-Term Care: Have knowledge of long-term care, community resources, and cost-effective alternatives regarding patient care delivery systems.
