Job Summary

Responsibilities
  • Manage a panel of patients with acute and chronic health conditions, focusing on health promotion and disease prevention using evidence-based practice.
  • Develop and implement individualized plans to achieve optimal population health and disease management.
  • Lead interdisciplinary teams and work collaboratively with primary care team members to deliver comprehensive care.
  • Order and interpret labs, tests, and diagnostic studies, and initiate referrals and consults to specialty services as needed.
  • Provide education to patients and their families, and demonstrate critical thinking in assessing and managing complex situations.
  • Incorporate research, evidence-based practice, and established professional, community, or regulatory standards to validate or change clinical practice.
Requirements
  • Master's or doctoral degree as a Nurse Practitioner.
  • Graduate of a school of professional nursing approved by the appropriate accrediting agency and accredited by one of the following accrediting bodies: The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE).
  • Current, full, active, and unrestricted registration and certification as an Advanced Practice Registered Nurse (APRN) in a State, Territory, or Commonwealth of the United States, or in the District of Columbia.
  • Basic Certification as an APRN from one of the following: the American Nurses Credentialing Center (ANCC), American Academy of Nurse Practitioners Certification Board (AANPCB), or other nationally recognized certifying body in the area of clinical specialty.
Preferred Qualifications
  • Extensive women's health experience.
  • At least 1 year of APRN experience.
  • Extensive primary care experience.
Working Conditions

This position requires heavy lifting, 45 pounds and over, heavy carrying, 45 pounds and over, straight pulling 1 hour, pushing 1 hour, reaching above shoulders, use of fingers, both hands, walking and standing 7 hours, kneeling half hour, repeated bending for 7 hours, both legs, ability for rapid mental and muscular coordination simultaneously, near vision correctable at 13" to 16" to Jaeger 1 to 4, far vision correctable in one eye to 20/20 and to 20/40 in the other, ability to distinguish basic colors and shades of colors, hearing without aid, mental and emotional stability, and working with detergents and antiseptic solutions.
