About the Role

We are seeking a highly skilled Nurse Practitioner to join our team at Illumia Health, a fast-growing part of Gentiva, an industry leader in hospice and palliative care. As a Nurse Practitioner in our Advanced Illness Management (AIM) Palliative Care program, you will play a critical role in providing comprehensive, individualized treatment plans to patients with advanced illnesses.

Key Responsibilities
  • Collaborate with our Professional Services Corporation Physician and other palliative team members to deliver advanced illness management and palliative care services.
  • Assess patients' needs and develop individualized treatment plans, including diagnosis, treatment, and follow-up care.
  • Work autonomously in an Advanced Practice Role, making independent decisions and taking responsibility for patient care.
  • Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination.
  • Participate in evening/weekend call as required, conducting on-call services in a clinically competent and responsive manner.
Requirements
  • Master's degree in Nursing from an accredited school of nursing.
  • Minimum of two years of Nurse Practitioner experience, with a specialty in Family or Adult Medicine preferred.
  • Current license as a registered nurse and Nurse Practitioner certification/license.
  • Collaborative agreement with a supervising physician, part of the Professional Services Corporation, to supervise work in advanced illness management and palliative care.
  • Part B Provider certification and Federal DEA license.
