Key Responsibilities:- Initiate palliative care consultations and develop treatment plans for patients in all settings, including history, physical examination, and medication management.
- Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination.
- Provide patient education, counseling, and referrals to community resources as needed.
- Conduct face-to-face recertification visits and gather data to support hospice eligibility.
- Participate in interdisciplinary team meetings and contribute to the development of patient care plans.
- Train and educate staff on palliative care best practices.
- Current unrestricted license to practice as a registered professional nurse in the state of employment.
- Graduate of an approved Nurse Practitioner program.
- Current unrestricted license to practice as a Nurse Practitioner by the Board of Nursing in the state of employment.
- Current and continued CPR certification.
- Certified Hospice and Palliative Care Nurse (CHPN) preferred.
- Minimum of 2 years of palliative care and/or hospice experience required.
