**About the Role**

We are seeking a skilled Transportation Assistant to join our team at Worthington Healthcare Center, a member of the CommuniCare Family of Companies. As a Transportation Assistant, you will play a vital role in ensuring the safe and timely transportation of residents to and from outside physician appointments and events.

**Key Responsibilities**

* Transport and assist residents to outside physician visits in a safe and efficient manner
* Assist with transporting residents within the facility to scheduled specialty appointments
* Provide one-on-one monitoring for residents deemed an elopement risk on outside visits
* Perform basic nursing care procedures for residents as required
* Maintain resident documentation records

**Requirements**

* High school graduate or G.E.D. equivalent
* Current, valid Driver's License in the State of employment
* Graduate of an approved Nurse Aide Training Program or eligible to enter such a program and graduate and pass a state test within 120 days from date of employment
* Red Cross First Aid Certificate
* Prior work/life experiences involving independent decision making
