Key Responsibilities:

  • Conduct regular face-to-face sessions with clients to discuss their progress and goals
  • Monitor clients' safety and well-being, and provide support as needed
  • Assist clients with medication adherence and life skills coaching
  • Engage in recovery-related activities with clients, such as exercise and meditation
  • Provide phone support to clients and maintain ongoing communication with the case manager
  • Document all sessions and interactions with clients

Requirements:

  • Familiarity with mental health treatment and services
  • Experience working with clients with mental health and addiction challenges
  • CPR certification and a valid driver's license
  • Strong problem-solving skills and ability to manage potential issues
  • Able to remain empathetic and professional in stressful situations
